Peer-Reviewed Paper in the Official Journal of the Association of Neurovascular Clinicians | May 9th, 2024

Feasibility of a Telemedicine-Based Principal Illness Navigation (PIN) Service for Complex Populations Following Hospital Discharge After Acute Stroke

ABSTRACT

Background

Principal Illness Navigation (PIN) services may play an important role in helping patients through important transitions in care following acute hospitalization. We evaluated a novel PIN telemedicine approach to understand the feasibility of providing these services to diverse patient cohorts.

Methods

A single-arm, retrospective observational study of Kandu Health’s post-acute PIN service was conducted in patients experiencing ischemic or hemorrhagic stroke in California and New Jersey. The technology-enabled program offered remote healthcare support led by occupational therapists and licensed clinical social workers that was tailored to individual patient needs to facilitate transition to community settings post-discharge. Barriers to recovery were addressed through patient education, one-on-one guidance, and specialized referrals. Patient outcomes were assessed through in-app assessments and clinician-assessed modified Rankin Scores conducted via video consultation. Readmissions were monitored through both patient reporting and admission/discharge/transfer feeds from health information exchanges.

Kandu Announces Preliminary Outcomes of the First 100 Patients

Published in: International Stroke Conference (AHA) | February 15, 2024

100 patient data demonstrated over a 50% reduction of inpatient readmissions compared to published ranges

In an ISC 2024 poster presentation on the first 100 patients enrolled in Kandu’s stroke recovery program, Dr. Nancey Trevanian Tsai and team demonstrated impressive improvements in 90-day modified Rankin Scores (mRS), along with inpatient readmission rates >50% lower than those observed in published literature. Ninety five percent (95%) of program participants achieved clinician-assessed mRS scores of 0-2 at 90 days.

Kandu offers a novel, holistic model for stroke survivors during the first months of their return to home and community settings. They partner with hospitals in the delivery of the program to encourage continuity of patient care and change the course of stroke recovery. The service includes a combination of one-on-one support from a licensed Kandu Navigator, multidisciplinary specialty consults, and education, resources, and community delivered through Kandu’s novel app and communication platforms.

Kandu Health, a digital health company based in California, has developed a platform to assist stroke survivors with aftercare. Healthcare for stroke patients is primarily focused on acute care to limit the damage caused by the stroke. However, the company has identified that stroke survivors are frequently underserved after hospital discharge, and in many cases are left to cope with the psychological and physical fallout of the stroke with minimal support.

Kandu Health: A Stroke Focused Digital Health Company Leans Into Regulation

Author: Mary Stuart – MedTech Strategist | Published: April 5, 2023

In focusing on patients in their first 90 days following a stroke, Kandu Health aims to help a population with unrecognized medical and functional needs, and in so doing, improve patient outcomes and the economics for all constituents who bear the costs of post-stroke care. But unlike many digital health companies, Kandu Health believes it provides the greatest value by embracing, rather than avoiding health system regulations.
